In order to comply with the Personal Data Protection Act and the Privacy Policy, the National Cheng Kung University Hospital (hereinafter referred to as the Hospital) hereby informs you of the following items in accordance with the laws before collecting your personal data. Please read carefully.

1. Purposes of Collection and Categories

For the purpose of carrying out the Hospital's business, activity or project, the purpose of providing service, internal administrative management or reporting to the competent authority, the Hospital needs you to provide these categories of personal data:

• C001 Type for identifying individuals

e.g. name, mobile phone number, e-mail address, postal address, and any other information by which individual can be identified.

• C002 Type for identifying finance

e.g. the number and name for an account at a financial institution.

• C003 Type for identifying in government data

e.g. the number of personal ID card.

• C011 Characteristics Information

e.g. age, gender and date of birth, etc.

2. Scope of Use

Unless the purposes of use relating to international businesses or activities, your personal data will be used solely by the Hospital in a reasonable way in the territory of the Republic of China to the extent necessary to implement the purposes of collection until the purposes of collection prescribed above is fulfilled.

3. Rights of Data Subject

In accordance with Article 3 of the Personal Data Protection Act, you shall exercise the following rights with respect to your personal informaton maintained by the Hospital:

• allow you to examine or read;
• provide a copy;
• allow you to modify;
• stop collecting, processing or using; and
• delete.

4. Impact on Rights and Interests of Not Providing Personal Data

If you provide incorrect personal data or choose not to provide your personal data, the Hospital may not be able to provide you with services relating to the purposes of collection.

5. You understand that this document complies with the R.O.C. Personal Data Protection Act and relating regulations, and you agree that the Hospital keeps this document for further checking.
